Life Insurance provides your named beneficiary(ies) with a benefit in the event of your death.
Accidental Death and Dismemberment (AD&D) Insurance provides specified benefits to you in the event of a covered accidental bodily injury that directly causes dismemberment (i.e., the loss of a hand, foot or eye). In the event that your death occurs due to a covered accident, both the Life and the AD&D benefit would be payable.
Basic Life and AD&D (Company Paid)
EAG provides Basic Life and Accidental Death and Dismemberment (AD&D) insurance through SunLife. EAG pays the premium for this important protection to provide comfort and security to your loved ones in the event an accident or death.
Your Basic Life and AD&D coverage amount: 1 times your base annual salary up to a $200,000 maximum.
Supplemental Life/AD&D (Employee-paid)
If you determine you need more than the Company-paid Basic Life/AD&D coverage, you may purchase additional coverage for yourself and your eligible family members.
Increments of $10,000 up to 5 times your annual salary or $500,000
Increments of $5,000 up to $500,000 (not to exceed 100% of your additional life coverage)
14 days to 6 months: $500
6 months to 26 years: increments of $2,000 up to $10,000 (not to exceed 100% of your additional life coverage)
14 days to 6 months: $500
6 months to 26 years: $10,000
*During your initial eligibility period only, you can receive coverage up to the Guarantee Issue amounts without having to provide Evidence of Insurability (information about your health). Coverage amounts that require Evidence of Insurability will not be effective unless approved by the insurance carrier.
If you’re unable to work because you are ill or injured, EAG provides Short-Term and Long-Term Disability Insurance to support you so that you can focus on recovering, instead of worrying about how to pay your bills. These benefits are provided at NO COST to you through SunLife and are coordinated with State disability programs where available.
EAG provides you the opportunity to purchase voluntary benefits through Allstate, ARAG and Nationwide at affordable group rates. The full cost of these benefits are paid 100% by you through payroll deductions.
These coverage options complement your medical plan by providing cash benefits that can be used to help pay for the out-of-pocket expenses your plan may not cover such as deductibles, coinsurance, and copays. Benefits are paid directly to you — not to a doctor or health care provider. You may choose to enroll in one, two or all three voluntary benefit plans.

With the Critical Illness plan, you have protection for you and your family to cope and recover from the financial stress of surviving a critical illness. Coverage is available for you and your family. As an insured, you will receive a lump-sum benefit of up to $10,000 upon initial diagnosis of the covered illness. You can use your benefit to recover from medical bills or for whatever else you may need. This plan can help you meet your financial obligations, such as medical bills, deductibles and uncovered expenses as well as mortgage payments and other ongoing living expenses.
